What are CCSP and SOURCE?

CCSP (Community Care Services Program) and SOURCE (Service Options Using Resources in a Community Environment) are Georgia Medicaid waiver programs that may help eligible seniors and adults with disabilities receive home and community-based services as an alternative to nursing facility care. Eligibility and covered services are determined by the State of Georgia, where applicable.

Who may qualify?

Program eligibility is set by Georgia Medicaid. People who may qualify generally include:

• Georgia residents who are Medicaid-eligible
• Seniors (typically 65+) or adults with qualifying disabilities
• Individuals who meet a nursing facility level-of-care need
• Those who can be safely supported at home with services in place

Covered support services

Depending on eligibility and the approved plan of care, supports may include:

• Personal support services (bathing, grooming, dressing, mobility)
• Homemaker services such as meal preparation and light housekeeping
• Respite care for family caregivers
• Companionship and supervision
• Care coordination with the case management team
